Request An Appointment

This form is a request. A clinician will call you to set up your appointment.

We might text you for clarification or to let you know when we will be calling if we are in a treatment or if it is after hours.  By submitting your phone number, you agree to receive calls and/or texts from Knoxville Lice Clinic to communicate with you about your request.

Please keep the following in mind when you speak with your clinician:

Make an appointment for each person you want checked or treated. If you do not make an appointment for each person, we may not be able to treat them at the same time as the rest of your family.

Please bring a person to watch young children while you are being treated.  Please bring a carrier for your infant.  We can’t treat anyone while they are holding a child.

Other than one person to help watch young children, please only bring people being checked or treated.

Call us if something changes. Emergencies happen, cars will break down, you might wake up sick. A quick call enables us to reschedule you for as soon as you can make it in, and lets us help another family dealing with lice in that time slot.  If you do not keep your appointment and do not cancel three or more hours prior to your appointment, we will be unable to refund your deposit.

Regretfully, if you are more than 15 minutes late we will have to cancel your appointment and you will forfeit your deposit.
